Step 3: You are to download and print the form, fill the form and return the form physically or through proxy (a person appointed by you) at the ICSN Office, PEI Centre building, 114B Ilupeju Street, Dolphin Estate, Ikoyi, Lagos with your full payment. Payment is via cash or POS terminal/ATM card at the centre.
